What slows down my laptop (Lenovo z460 i5)?

I need help. I'm having problems with my laptop recently. It's very slow during boot. And it hangs every time a click on something (like folders, start button, computer, etc.). Been using this laptop for 3 years now. I'm not sure what causes the problem. Is it because of a faulty hardware? I wasn't having these problems before.

Added (1). And by the way, I already did a CHKDSK. And reformat and re-installed windows 7 (also tried windows 8). I'm still having the same problems.

Could be hardware or software related.

On the software side do the following:

Perform a CHKDSK first (type CHKDSK C: /F at the CMD Prompt) and create a Restore Point

Clean temps using TFC by Oldtimer

DO a full scan with an antivirus and antimalware. I suggest you use Qihoo 360 TS and Malwarebytes

Uninstall unnecessary apps with PC Decrapifier

1) Scan it with SeaTools for DOS & Memtest86+.
2) Then scan Windows with SuperAntiSpyware & Malwarebytes and remove anything they found.
3) Uninstall all useless software.
4) Disable all useless startup software with CCleaner but don't reg clean it.
5) Defrag the hard drive.
6) Uninstall any anti-virus software and use MSE.
7) Use Chrome or Firefox with security plugins like WOT, Bluhell Firewall, Adblock Plus, NoScript etc.
8) Run any shady websites & software within Sandboxie.
9) Run "sfc /scannow" in "cmd". If Windows is corrupted, reinstall it.
