So what is the least amount of work I can do to improve my PC's capabilities?
Can I just get a better graphics card and more memory or do I need to improve other components all together?

you need a intel i5 to i7 cpu for modern gaming
First, do a tune up and uninstall junk programs that hog memory.
Video card would give you the biggest increase by far

If it isn't a laptop, then I would look for a video card, but you have to make sure your power supply can provide enough power for a new video card.
32mb is what my older dell latitude has as dedicated video ram even if you put a good gpu and a require psu in the system some bottlenecking might occur running some games, i would suggest a upgrade of most parts if into a lot of modern gaming and having it run smoothly
